Premium Direct Care Package

Includes:
Full access to Dr. Ramzan - You will have his personal cell phone # for urgent issues during evening and weekend hours. You also will have his personal email address for non-urgent issues, questions, updates, or advice.
Executive Physical - 90 minutes. A thorough Diagnostic Physical Exam will be performed, Vitals, Pulse oximetry, EKG (cardiac testing), Stool guaic for occult blood, Urinalysis, Respiratory Spirometry with Peak Flow and FEV1 Calculations, Vision, Hearing, Screening for Cancer: Testicular, Prostate, Colon Ca (Male) Breast and Cervical (Female if not done by Ob/gyn). Full cardiac risk stratification, Full preventative maintenance checklist based upon age. Sendout blood tests (covered by your insurance) including but not limited to Fasting lipids, Glucose, CBC, Basic Metabolic Panel, TSH. And finally a detailed open discussion about health status and an open question/answer session. For those who do not have insurance, we can negotiate inclusion of certain blood tests.
Unlimited Medical Visits for any general health or acute urgent care reasons. 30-60 minutes can be allotted if necessary. Same day appointments, extended evening hours, or weekend appointments can be arranged if necessary.
Procedures: for acute issues or minor surgical intervention (laceration/cut suturing, incision and drainage of abscesses, wart/skin tag removal, skin biopsy, foreign body/bug/tick removal, ear wax removal, fluorescein eye exam, wound care, minor burn care, sprains, minor fractures.
Elective Flu shot offered.
Medication Injections for: Pain, Nausea, Allergy, Infection if indicated. Steroid injections into joints/tendons/muscles/trigger points (maximum 2 per year)
Respiratory Nebulizer treatments
In-House Lab tests: Rapid strep, Rapid Flu, Mono, Urine pregnancy, Glucose finger stick, Hemoglobin finger stick, PPD TB screening

Hospital Care and Guidance: If you get admitted to the Hospital for any reason, I will be there for you. I am active staff and have attending admitting privileges at local hospitals. If you need to be admitted to the hospital, I will facilitate the process and personally try to get you a direct admission (if possible) without the pain and hassle of going through the E.R. I will come and visit you daily in the hospital and help coordinate your care as long as you are hospitalized. I will be in close daily touch with the hospitalists and specialists involved in your care. I will be your patient advocate and explain to you all of your tests, diagnosis, treatments, and help you make informed decisions about your medical care. You will have a guide to navigate you through the confusing maze and culture of the hospital. I will ensure your discharge planning from the hospital is done correctly and you are given all the resources and support needed. With your permission, I will be the liaison between all the doctors, nurses, and other ancilliary staff.
Home visits: If necessary, I can arrange a medical visit to your house like the “old-fashioned” Physicians did in the past.
Visits to Workplace or Business: The Doctor can come to you during your busy day and have a medical visit scheduled into your workday at your office.
